Please review to be sure that you quality for funds from the Angel Fund
Qualifications:
1. You must be a resident of Carroll County.
2. You must have no more than 3 pets.
3. You have applied for, and been denied for CareCredit and have a CareCredit #.
4. You must have a treating Vet participating in the Angel Fund.
You do NOT qualify if:
1. You are requesting funds for routine care such as spay / neuter and vaccines.
2. You have been approved for CareCredit.
3. Your Veterinarian has agreed to work with you on a payment plan.
4. You are a breeder or rescue group.
5. You are seeking help for an elective surgery
6. Your pet has been diagnosed with an terminal disease.
If you feel that you qualify for financial help from the Angel Fund based on this criteria, please fill out the application with the information related to your pet. Your treating Vet will be required to verify your request and information.
